In order to begin your membership you will be asked to sign this agreement and you will receive a copy for your own records.

1) First month payment is required for sign-up for any membership and is not refundable or transferable. The first credit is available for you to use immediately.

​

2) Membership commences on the date of sign-up and the amount will be charged each month on that date. If you need to modify your billing date please reach out any time. If you schedule a subsequent appointment before your billing cycle date, your billing date will change to that new date. For example: if your original billing date was April 17th and in May you would like to book your appointment for the 6th, your billing date would change to the 6th of each month starting on the day of your appointment.

3) A credit/debit card must be used and is kept securely on file for automatic payment of monthly membership fees. If there is any issue with a payment it is the member's responsibility to rectify this issue as soon as it is discovered. The Soma Haus LLC is in no way, responsible or obligated to pay for a member’s insufficient fund fees.

4) All membership fees are nonrefundable once charged.

​

5) Unused membership credits roll over 3 times (3 months) before they expire.

​

6) Memberships may be cancelled at any time with no fee. If there are any unused credits at the time of cancellation, they will remain available on your account for the standard time as described in #5. Once your membership is cancelled you are not eligible to restart a new membership for a minimum of 6 months, and if there is a waitlist you may choose to be added to it.

7) Members may opt to pause their membership at any time. If you are unable to use your membership for an amount of time exceeding 3 months you are permitted to freeze your membership. Memberships may be paused for a minimum of one month and a maximum of three months (beyond the three month grace period where credits roll over). During the pause time period no additional charges shall be made and no new credits will be accrued. If a member opts to pause for the maximum of 3 months and is unable to resume their membership, their membership will be cancelled at no fee and any remaining credits will expire and are not eligible for refund. A maximum of one pause per year, and two pauses in a five year period are allowed for any one member. Memberships may be temporarily paused under the following conditions:

• ​member is undergoing surgery that requires more than 3 months of rehab/recovery in which massage therapy or Manual Lymphatic Drainage is contraindicated

• member has a temporary health condition for which massage therapy or Manual Lymphatic Drainage is contraindicated

• member is hospitalized for more than 3 months

• member is travelling for a period of time exceeding 3 months

 

​8) Memberships are for one person only, they may not be shared. Membership credits may not be transferred to another person.

​

9) Membership services are not eligible for any additional savings (coupons, discounts, promotions, etc.)

​

10) Members are asked to provide a minimum of 24 hours notice for any appointment change or cancellation. Please text Wren directly as soon as possible 312-438-8547. If a member is a no call/no show without a reasonable excuse for their scheduled appointment they will forfeit the credit and it will not roll over. If a member repeatedly cancels or reschedules their appointment with less than 24 hours notice they may be subject to additional fees or membership cancellation at the discretion of the business.

11) Members receive the following benefits:

• discounted monthly massage

• unlimited add-ons at no additional cost

• priority booking (recommended booking 2-3 months in advance)

• 10% off all regular-price retail/products

• members may opt to schedule an additional service outside of their membership for themselves (not for others), for this additional service they will enjoy 15% off any service under $100, and $15 off any service $100 and over

• annual member appreciation gift

• membership sign-up gift

12) The conditions of the membership agreement are subject to change at the discretion of the business, and the member shall receive reasonable advanced notice of any changes.
